Who approves OMMA licenses?

The OMMAis responsible for receiving and reviewing all OMMA licensing applications. Initially established under the Oklahoma State Department of Health (OSDH) following the passage of State Question 788, OMMA became an independent state agency in 2022 through Senate Bill 1543. OMMA continues to oversee the state’s medical program.

How long does it take for a patient license application to be approved?

OMMA is required by state law to process patient license applications within 14 business days from the submission date. Applicants will receive an email notification after their application is reviewed. For approved applications, it takes an additional one to two business days to print the patient license identification card, which is then mailed via regular U.S. Postal Service first-class mail.

If an application is incomplete due to unclear or missing information or documents, OMMA will send an email detailing the issues. Applicants can log back into their account to make corrections upon receiving this email.

If an application is denied because requirements have not been met, OMMA will mail a denial letter stating the reasons within 14 business days of submission.

How much does an OMMA patient license cost?

The standard application fee is $100. Applicants who provide acceptable proof of enrollment in Medicaid (SoonerSelect), Medicare, or status as a 100% disabled veteran are eligible for a reduced fee of $20. Application fees must be paid using Visa, MasterCard, or Discover credit or debit cards, with an additional credit card processing fee applied. These fees are non-refundable, even if the application is not approved.

How do I get a physician’s recommendation for an adult OMMA patient license?

Schedule a visit with a physician licensed in Oklahoma to discuss your condition. If the physician determines that you may benefit from physician-recommended products, they will complete and sign the Adult Patient Physician Recommendation Form. A digital copy of this form must be submitted with your online application within 30 days of the physician’s signature date.

What do I need from a physician to complete an OMMA patient license application?

Applicants aged 18 and older must obtain a signed Adult Patient Physician Recommendation Form. Applicants under 18 require two physicians’ signatures using the Minor Patient Physician Recommendation Form.
