Wondering what’s needed to renew your OMMA Patient Card in Oklahoma? OMMA Renewals are simpler than ever with our telemedicine service.

According to the OMMA, the following items and steps are needed:

1. New Physician Recommendation Form (two signed forms for minor applicants)
2. Proof of Identity (driver’s license, passport or approved tribal ID)
3. New Photo (digital passport-style photo taken within last 6 months)

Renewal applications can not be submitted more than 30 days before the current license expires.
